Bayside Health invites applications for a Social Work Grade 3 position within the Alfred Trauma Service. The role requires hospital-based experience, strong discharge planning, and psychosocial interventions to support patients and families.

Located at The Alfred, it is a fixed-term, full-time role with weekend work once per month. Applicants should hold a Bachelor Degree in Social Work and have experience with EMR systems, NDIS, aged care and VPTAS processes.

Qualifications

The Department

The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on quality of life and participation in care is a primary goal of the Department.

Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home.

Social Work provides a range of services including:

• Counselling
• Crisis interventions
• Bereavement counselling
• Family support
• Care coordination
• Making Appropriate referrals
• Community liaison

The Role

A fantastic opportunity exists for an experienced Social worker to work in the Trauma Service at The Alfred. You will have demonstrated prior experience in managing a complex clinical caseload and will have sound knowledge and skills in hospital discharge planning and providing complex interventions to facilitate discharge. An understanding of grief and loss counselling and psychosocial support is also essential. You will possess strong assessment, case planning and have ewxperience in providing staff and student support. You will work within a strong multidisciplinary team and will be part of an organisation where professionalism, innovation and the ability to find creative solutions to challenges is highly regarded. You will be required to work in any of the units within the Trauma stream as directed by the Trauma Team leader. This role will require the applicant to provide leadership to the Trauma team and the SW department when required in collaboration and daily consultation with the Grade 3 Trauma Team Lead.

The role will involve providing comprehensive psychosocial assessments, crisis intervention, adjustment counselling, therapy, case planning, family work, community liaison, networking and referral to external stakeholders and discharge planning.

The successful applicant will need to contribute to quality improvement and research activities and provide staff and student supervision. This role will also require the applicant to work one weekend day per month.

Experience and Qualifications Required

• Applicants must hold a Bachelor Degree in Social Work
• Applicants must have an understanding of the psychosocial issues associated with this patient cohort
• Substantial experience in working within a hospital setting will be essential to this role

It is vital that the successful candidate has experience in working within an Electronic medical record. A knowledge of NDIS, Aged care and VPTAS processes is essential for this role.
