Job summary

NSW Health Westmead Hospital is seeking a Senior Social Worker to support renal patients and families through complex psychosocial care. You will provide specialist assessments, counselling and practical support using a psychosocial approach to help navigate difficult decisions with dignity and choice.

You will work across Renal and Renal Supportive Care, collaborating with medical, nursing and allied health teams to coordinate holistic care and contribute to education, supervision and quality

Qualifications

• Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW).
• Extensive specialist knowledge in Renal and/or Palliative Care Social work including advanced care planning and grief support.
• Experience in staff and/or student supervision; skills in policy development and service improvement.

Responsibilities

• Provide autonomous, high-level social work intervention across all stages of Renal Supportive Care.
• Support advance care planning, guardianship, wills, funeral planning and access to relevant services.
• Participate in multidisciplinary case conferences, clinical reviews, ward meetings and education programs to promote integrated care.
• Advocate for patients and coordinate services to respond to complex psychosocial needs.
• Contribute to education, supervision, research and quality improvement while supporting students.
