Job summary

Queensland Health's North West Hospital and Health Service seeks an experienced social worker to provide advanced clinical services to patients with chronic kidney disease in Mount Isa and surrounding communities.

You will lead, supervise and mentor staff, coordinate psychosocial care, advocate for patients, and work with multidisciplinary teams across Mount Isa, Cloncurry, Doomadgee and Normanton to improve outcomes.

About the role

You will provide advanced social work services to patients receiving renal care across North West Queensland.

• Deliver high-level social work services, psychosocial assessments, counselling, and care coordination for patients with chronic kidney disease and their families.
• Provide clinical leadership, supervision, mentoring, and professional support to staff and students while driving continuous service improvement.
• Advocate for patients and facilitate access to appropriate community, health, and support services across the care continuum.
• Partner with multidisciplinary teams to improve patient outcomes across Mount Isa and outreach locations including Cloncurry, Doomadgee, Normanton, and Mornington Island.

Working with us

• Eligible health practitioners entitled to accrue an additional three days professional development leave per annum, resulting in a total of six days professional development leave per annum.
• Eligible health practitioners working in designated remote areas will be entitled to a professional development allowance of $3,512.00 per annum.
• Rural and Remote incentive: Category D allowance of $431.22 p.f.

Tax savings through

• Eligible health practitioners entitled to accrue an additional three days professional development leave per annum, resulting in a total of six days professional development leave per annum.
• Eligible health practitioners working in designated remote areas will be entitled to a professional development allowance of $3,512.00 per annum.
• Rural and Remote incentive: Category D allowance of $431.22 p.f.

Tax savings through salary sacrificing options are also available.

Requirements

• Potential applicants are advised that the Commission for Children and Young People and Child Guardian Act 2000 requires Queensland Health to seek a ‘working with children check; from the Commission for Children and Young People and Child Guardian prior to appointment to this position.
• Mandatory requirements are a relevant tertiary degree qualification and appropriate registration/licences/memberships.
• C Class Motor Vehicle Licence.
• Pre-employment screening will be conducted for the successful applicant, including Criminal History checks.
• This is a VPD risk role and may require evidence of vaccination or immunity to: Hepatitis A/B, Measles, Mumps, Rubella, Varicella, Pertussis, and Tuberculosis.
• A period of probation may apply.
