Job summary

St Vincent's Private Hospital Brisbane seeks an experienced Grade 2 Physiotherapist to join the Rehab at Home team in Werribee, South West Melbourne. This role delivers hospital-level rehabilitation in patients' homes, combining hands-on assessments with virtual health technologies and multidisciplinary collaboration.

You will manage your own caseload across Melbourne with autonomy, supported by a skilled team, using digital health tools to tailor evidence-based rehabilitation and promote faster

Qualifications

• Must have current AHPRA registration.
• Minimum three years post-graduate physiotherapy experience.
• Experience in rehabilitation, orthopaedics, subacute or community care.
• Strong clinical reasoning and evidence-based practice.
• Excellent communication, organisation and time management.
• Ability to work autonomously in community settings.
• Experience with digital health platforms or telehealth.

Responsibilities

• Deliver home-based physiotherapy assessments and rehabilitation programs.
• Support patients recovering from elective orthopaedic surgery and hospital admissions.
• Develop personalised treatment plans to maximise recovery, mobility and independence.
• Utilise virtual care and digital health technologies to enhance continuity of care.
• Collaborate with medical, nursing and allied health professionals to optimise outcomes.
• Support early discharge and hospital avoidance initiatives.
• Maintain accurate clinical documentation and contribute to quality improvement.
• Champion patient-centred, evidence-based rehabilitation practice.
