Central Adelaide Local Health Network is seeking a Senior Pharmacist for the Transition Care Program in Adelaide. You will lead a pharmacy team, provide high‑level clinical services and contribute to improving outcomes for older patients transitioning from hospital to home.
You will oversee medication histories, safety documentation and audits while collaborating with medical and nursing staff to ensure cost‑effective, safe pharmacotherapy across Residential Care Facilities.

The Transition Care Program (TCP) is a Commonwealth funded program which supports elderly patients to regain function, strength and independence in order to return home post an acute hospitalisation which has resulted in a loss of their functionality.
TCP provides a restorative service via specialist assessment and management for patients 65 years and over. The pharmacist works with a multi-disciplinary team to provide education to clients in residential TCP Aged Care Facilities. The pharmacist also provides an accurate medication history and analysis of client journey from admission to discharge. The Pharmacist attends weekly multidisciplinary case management meetings on site at each Residential Care Facility. They also are responsible for completing the medication safety documentation and auditing requirements essential for accreditation.
Responsible for the organisation, management and operation of the designated pharmacy area. The provision of a comprehensive high-level range of pharmacy services, to exercise independent and autonomous judgement in the provision of pharmacy services, medicines information and therapeutic advice to patients of the hospital as well as medical and nursing staff and other health professionals in accordance with hospital and divisional policies and procedures and professional standards and guidelines relating to pharmacy practice. The optimisation of outcomes for patients by implementing strategies to achieve the best possible quality use of pharmaceuticals in a safe and cost-effective manner and in accordance with hospital policy.
You're an experienced hospital pharmacist who enjoys leading teams, improving services and delivering exceptional patient care. You'll bring strong clinical expertise, excellent communication skills and a collaborative approach to supporting patients and healthcare teams.
