Qualifications

• Registered nurse with Nurse Practitioner status and current registration.

Responsibilities

• Coordinate care for service users through triage, assessment and safety planning.
• Develop and monitor care plans with multidisciplinary teams.
• Lead risk assessments and formulate risk management plans.
• Ensure safeguarding requirements are followed and attend relevant meetings.
• Provide clinical and managerial guidance to team members.
• Contribute to quality improvement and evidence-based practice.
