Job summary

Sussex Community NHS Foundation Trust Frailty Practitioner role within Integrated Neighbourhood Teams across Sussex offers advanced clinical care to people living with frailty. You will assess, plan and coordinate personalised management with GPs, community nurses, therapists and social care.

You will manage a defined caseload, identify deterioration early and support safe discharge while contributing to service development and education.

Qualifications

• Current registration as a Registered Nurse (Adult) or eligible Allied Health Professional.
• Independent prescribing qualification or plan to complete within 12 months of appointment.
• Band 6 level experience or above in frailty or elderly care.
• Proven ability to work autonomously within multidisciplinary teams.

Responsibilities

• Undertake comprehensive geriatric assessments and develop personalised care and support plans.
• Manage a defined caseload of people living with frailty and complex needs.
• Use clinical judgement to identify deterioration and take action to prevent avoidable hospital admissions.
• Coordinate care across primary, community, acute, social care and voluntary sector services.
• Support safe and timely hospital discharge, reducing readmission risk.
• Provide specialist clinical advice, leadership, education and support to colleagues.
• Contribute to service development and integrated care pathways.
• Advocate for people living with frailty and their families to maintain independence.
