Job summary

Monash Health, Clayton, VIC, is seeking a Clinical Nurse Consultant in Palliative Care to join the Supportive & Palliative Care Consultancy Service on a Full Time Fixed Term (and/or Part Time Fixed Term) basis. The role focuses on advanced clinical practice, leadership and education across Monash Health campuses.

You will assess and manage patients with complex palliative needs, coordinate treatment plans and mentor staff to build palliative care capability.

Qualifications

• Current registration with AHPRA as a Registered Nurse, essential.
• Extensive experience in palliative care and/or oncology, essential.
• Advanced clinical assessment, communication and care coordination skills.
• Completion of, or progress towards, a postgraduate qualification in a relevant specialty, highly desirable.
• Current Victorian driver’s licence and ability to work across Monash Health campuses.

Responsibilities

• Providing comprehensive palliative care assessment, consultation and symptom management for patients with complex needs.
• Supporting patients and families through goals of care discussions and psychosocial care.
• Coordinating multidisciplinary care and facilitating referrals to community and specialist palliative care services.
• Delivering education, mentoring and clinical support to nursing and multidisciplinary staff.
• Leading and contributing to quality improvement initiatives, research, audit and evidence-based practice.
• Providing consultancy services across multiple Monash Health campuses.
