A community care organization is seeking an enthusiastic Vietnamese speaking Senior Case Manager or Registered Nurse. The role involves assessing and planning client needs, providing support to families, and liaising with various organizations. Applicants should have a diploma in community care and be fluent in Vietnamese and English. Apollo Care Services is a provider of care services for both the Aged and Disabled. We are community driven and our goal is to provide holistic care and empower those who are less fortunate with both the education and tools to have a higher quality of life.
We are looking for an enthusiastic Vietnamese speaking Senior Case Manager or Registered Nurse to further develop and grow our business whilst also empowering our broader local community.
Your attention to detail driving the delivery of high customer service excellence with the ability to assist consumer budgets across different packages in either the disability or aged care sector.
Based: North of the river, Perth.
Assessing clients' needs and planning, developing and implementing educational, training and support programs
Interviewing clients and assessing the nature and extent of difficulties
Monitoring and reporting on the progress of clients
Referring clients to agencies that can provide additional help
Assist in-home assessments, referrals, care plan reviews & goal planning for consumers
Assessing community need and resources for health, welfare, housing, employment, training and other facilities and services
Liasing with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and services
Supporting families and providing education and care for children and disabled persons in adult service units, group housing and government institutions
Preparing submissions for funding and resources, and reports to government bodies and other agencies
Assisting young people to solve social, emotional and financial problems
Excellent time management with client case load
Respond & communicate effectively with clients re: changing care needs
Possess empathy & understanding towards the needs of older people
Working within a broader team and liaising with our expert staff
To work within a supportive disability/home care team of dedicated professionals

Bilingual - Fluent Vietnamese and Professional Working English at the minimum
Knowledge of & enthusiasm for Disability and Aged Care Services & a Consumer Directed care environment
Diploma level or above qualification in community, aged, health or another discipline desirable
Access to a reliable, registered motor vehicle with full comprehensive insurance and a valid driver's license
A clear national police check and working with children check upon role offer
