St Vincent's Private Hospital Brisbane – Melbourne VIC is seeking an ACA/ACCS Manager to lead the delivery of Aged Care Assessment and Aged Care Consultancy Services across hospital and community settings. You will oversee MAC assessments and ensure exceptional client-centred service delivery, coordinating across acute, subacute, palliative care and community services.

Qualifications

• Tertiary Allied Health or Registered Nurse qualification.
• Experience in health, aged care and ACA.
• Sound knowledge of ACA and service planning.
• Eligibility for membership of relevant Allied Health/nursing professional association and/or current registration/accreditation.
• Current National criminal history check, or willing to obtain/ Current NDIS Worker Screening Check, or willing to obtain.

Responsabilités

• Lead and support the multidisciplinary ACA and ACCS team across Fitzroy and Kew.
• Oversee the coordination of MAC assessments across acute, subacute, palliative care and community settings.
• Partner with internal teams, community providers and Lead Providers across the Vic 1 and Vic 2 aged care assessment regions.
• Drive service excellence, quality improvement and compliance with the aged care standards and best practice.
• Maintain a clinical caseload (as required) to support service delivery and operational priorities.
