Job summary

FYI Aged Care Services seeks a Clinical Care Manager (RN) to join our leadership team, guiding clinical services, governance and home-care delivery while developing systems and budgets for person-centred care.

The role offers flexible hours, with approximately one day per week initially and potential to grow, closely working with the CEO and team to ensure compliance with Aged Care Act and quality standards while building capability across the organisation.

Qualifications

• Current, unrestricted registration as a Registered Nurse with AHPRA.
• Experience in aged care, preferably within community aged care or home care.
• Strong clinical assessment, care planning and documentation skills.
• Knowledge of the Aged Care Act 2024, Aged Care Rules 2025 and quality standards.

Responsibilities

• Provide clinical leadership to ensure safe delivery of home care services.
• Conduct and review clinical assessments, identify needs, risks and goals.
• Develop, implement and review individualised care plans and budgets with consumers.
• Coordinate clinical care for consumers with varying complexity.
• Support the establishment of clinical governance, policies and procedures.
• Contribute to incident management, risk assessment and continuous improvement initiatives.
