Job summary

MacKillop Family Services in Warrnambool, VIC, seeks a dedicated Case Manager for our Residential Care Program. This permanent part-time role (3 days/week) reports to the Residential Coordinator and collaborates with workers, families, and external partners to coordinate case plans and advocate for young people’s needs.

You will work to build trust, promote safety, and empower young people toward positive futures, contributing to a supportive team and a culture of continuous service improvement.

Qualifications

• Valid and current Driver’s Licence.
• Satisfactory Criminal History Check (paid for by MacKillop).
• Valid Employee Working with Children Check.
• Valid and current NDIS Workers Screening Check (risk assessed / client facing roles). MacKillop can assist.

Responsibilities

• Support children and young people living in residential care to achieve their goals and reach positive outcomes.
• Develop and review case plans that reflect the individual needs, strengths, and aspirations of young people.
• Coordinate services and advocate for young people to ensure they have access to the support they need.
• Build positive relationships with families, carers, community services, and other key stakeholders.
• Facilitate care team meetings and contribute to planning and decision-making processes.
• Maintain accurate records, reports, and case documentation.
• Promote the safety, wellbeing, and rights of children and young people.
• Contribute to a positive team culture and continuous service improvement.
• Support a safe workplace by following organisational policies and health and safety practices.

MacKillop Family Services is looking for a dedicated Case Manager to join our Residential Care Program in Warrnambool. Our residential homes provide a safe and caring environment for young people who have experienced abuse, neglect, trauma, and separation from family. Through a therapeutic approach, we support young people to heal, build resilience, and create positive futures.

As a Case Manager, you'll work alongside young people, Residential Care Workers, families, and other professionals to ensure each child receives the support they need to thrive. You'll coordinate case plans, advocate for young people's needs, connect them with services, and help create opportunities that support their wellbeing, development, and goals.

We are recruiting a Case Manager based in Warrnambool, VIC within our Residential Care Program. The role reports to the Residential Coordinator and works closely with Residential Care Workers, leadership teams, families, and external stakeholders.

This is a permanent part-time position working 3 days per week (Work Hours are negotiable)!
