Job summary

Care Connector (Holistic Care Solutions) is a clinician-led provider delivering holistic, person-centred care across the community. We specialise in complex care and support participants to live safely and independently in their homes.

We are seeking Enrolled Nurses and experienced Disability Support Workers for current and upcoming opportunities across the Northern Gold Coast and Logan regions. This is not high-volume agency work; you will work with consistent participants, support complex and

Qualifications

• Current AHPRA registration is required for Enrolled Nurses.
• Experience in community or complex care is preferred.
• Experience with spinal cord injuries highly regarded for nursing roles.
• Certificate III/IV in Individual Support (or equivalent) is required for Support Workers.
• Experience supporting participants with complex needs is desirable.
• Current National Police Check, or willingness to obtain one.
• Must have work rights in Australia.
• NDIS Worker Screening Check + Blue Card.
• Availability for flexible shifts (including weekends if required).
• Experience working with complex behavioural management cases.

Responsibilities

• Deliver safe, person-centred care to participants with complex needs.
• Assist with personal care, daily living and community access.
• Follow care plans and positive behaviour support strategies.
• Monitor and report changes in client condition.
• Complete documentation (progress notes, incident reports).
• Build strong, professional and respectful relationships with clients.
