Job summary

The Far West Local Health District of NSW Health is seeking a passionate Palliative Care Registered Nurse to join the Specialist Community Palliative Care Team. You will deliver patient-centred palliative care across settings, coordinating with primary care, hospital and community services to support families and carers.

Qualifications

• Current Authority to Practice as a Registered Nurse with AHPRA with recent clinical experience relevant to palliative care pain and symptom management.
• Demonstrated ability to practice in accordance with RN Standards of Practice, Code of Ethics for Nurses, and Code of Professional Conduct for Nurses (Nursing and Midwifery Board of Australia).
• Demonstrated effective clinical communication through information technology, written and verbal means; and sound interpersonal skills including the ability to communicate with a diverse range of people in a sensitive manner.
• Demonstrated ability to work independently and as part of the multi-disciplinary care team; ability to prioritise and organise workload, and demonstrated leadership qualities.
• A demonstrated understanding of the challenges associated with rural and remote palliative care community nursing.
• Demonstrated commitment to quality improvement, practice development and evidence-based practice.
• Current Drivers licence and willingness to travel within the LHD.

• The Palliative Care Registered Nurse will work as a member of the Specialist Community Palliative Care Team providing patient centred palliative care for people with advanced life limiting illnesses, their families and carers.
• The role will provide a combination of direct/shared/consultative care across settings, in liaison with other FWLHD palliative care health professionals, and is expected to function as a member of a multidisciplinary team including primary care, specialist services, community based services, inpatient, aged care and volunteers.

This position qualifies for an annual Recruitment Incentive Package up to the value of $10,000 (conditions apply and may be pro-rata’d to your contracted or ordinary working hours, these payments are paid on a fortnightly basis) under theNSW Health Rural Health Workforce Incentive Scheme.

This scheme recognises the role key health workers play in enabling the delivery of rural and remote health services, and aims to provide incentives over and above award entitlements, to attract and retain staff in these areas.
