NSW Health's Sydney Local Health District seeks a Registered Nurse to join the Mobile Assertive Treatment Team in Community Mental Health Services. This temporary part-time role (24 hours per week) delivers community-based mental health care and coordination.

You will work with the Core Team to provide intake, assessment and treatment, upholding recovery-oriented practices and consumer involvement, with a NSW Driver's Licence required.

Qualifikationen

• Current nursing registration with AHPRA.
• Demonstrated competencies to provide contemporary mental health practice.
• Understanding of and commitment to recovery oriented services and consumer involvement in service provision.
• Current NSW Driver's Licence (P1 and P2 acceptable).

• Provide mental health clinical services including intake, assessment, treatment and care coordination.
• Care coordination is a main component of the model of care, including extended care coordination and short-term packages.
• Identify and plan short- and longer-term interventions; coordinate with GPs and other providers.
• Support discharge planning or refer back to primary care after intervention periods.

About the Role

Sydney Local Health District’s Community Health Centres have a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities.

The "Community Mental Health Team" (Core Team) is the mainstay of the community mental health system. Core Teams and provide a range of clinical services including intake, assessment, consultation, treatment ,and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care, and short term care coordination.

Consumers under the care of Core Teams will either have:

• Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months.
• A period of treatment as recommended by evidence based practices.
• Ongoing care and monitoring for prolonged periods.

The Mental Health Nurse will practice in keeping with the standardised Sydney LHD position description for the profession in addition to this position description.
