Job summary

Pearl Home Care Pty is seeking a compassionate Registered Nurse for a part-time role in Geelong and Ballarat, VIC. Based onsite at the Geelong office, you will conduct initial and ongoing nursing assessments, home visits and develop individualized care plans for clients in their homes and community settings.

The role emphasises evidence-based practice, liaising with Case Managers, GPs and families, and maintaining accurate clinical records.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• Current Australian driver’s licence and reliable vehicle.
• Excellent time management and organisational skills.
• Strong critical thinking and clinical judgement.
• Able to work independently and manage a caseload.
• Proficiency in wound care and medication administration.
• Commitment to confidentiality and professional ethics.
• Current First Aid certificate or willingness to obtain one.
• Immunisation records current as per aged care requirements.

Responsibilities

• Complete comprehensive initial and ongoing nursing assessments.
• Complete home visits and clinical reviews.
• Develop, review and update individualized care plans.
• Complete Personal Care and ADL assessments where required.
• Complete falls risk assessments, post-fall reviews and interventions.
• Identify and respond to deterioration or changes in a client’s health or functional status.
• Complete clinical risk assessments and risk-management plans.
• Escalate clinical concerns promptly and implement follow-up actions.
• Complete progress notes, nursing notes and clinical documentation.
• Maintain complete and up-to-date client clinical records.
• Review client documentation and ensure clinical information remains current.
• Complete incident assessments, investigations, follow-up and documentation.
• Assess incidents for potential SIRS reporting requirements and assist with SIRS management and reporting.
• Develop corrective actions and preventative strategies following incidents.
• Liaise with Case Managers/Care Partners regarding changes in client needs, risks and clinical recommendations.
• Liaise with GPs, hospitals, allied health professionals, families and other health professionals.
• Make appropriate clinical referrals and recommendations.
• Participate in case conferences and multidisciplinary care planning.
• Provide clinical guidance and support to support workers and other staff.
• Support safe delivery of personal care and other clinically related services.
• Monitor clients with complex or changing clinical needs and arrange appropriate follow-up.
• Ensure clinical practice and documentation meet Support at Home, Aged Care Quality Standards and organisational requirements.
