Job summary

Homewood Care Pty Ltd is seeking a Registered Nurse (RN) to deliver individualised, holistic care in alignment with Aged Care Safety and Quality Standards. You will lead and coordinate a multidisciplinary team, provide direct care, and maintain strong relationships with residents and families, while ensuring continuous improvement in care delivery.

Qualifications

• Must be a Registered Nurse with current AHPRA registration.
• Demonstrated clinical competency and currency as an RN.

Responsibilities

• Assess residents' complex care and risk factors during initial admission and set directives for ongoing care with the resident or nominated representative.
• Undertake and document evidence-based assessment promptly and clearly as care needs change.
• Assess residents for complex care needs and initiate referrals as needed.
• Plan with residents and staff goals and care related to complex health needs.
• Provide skilled nursing care and guidance to staff according to care plans.
• Perform medication administration, wound care, pain assessment and management, behaviour management and restraint management.
• Communicate effectively with residents, relatives, staff and other health professionals.
• Evaluate responses to interventions and adjust care with GP input when appropriate.
• Supervise and direct on-the-job safety and health of staff.
• Lead the clinical team during shifts and represent executive after-hours when RN in charge.

Job description

The role of the Registered Nurse (RN) is to ensure effective, individualised care that is assessed, planned, delivered, documented and evaluated for improvement opportunities for every resident's holistic care in accordance with the Aged Care Safety and Quality Standards. You will provide direct care delivery; effective leadership and management of staff that report to the RN including Care Workers (AIN/PCW), Team Leaders (TL); and coordination of care with the multidisciplinary care team (doctors, specialists, allied health professionals, Leisure and Lifestyle and hospitality staff). You will build and maintain relationships with each resident that benefit the resident, in collaboration with nominated representatives/friends and family, the Homewood Care team and health care providers.

Key responsibilities

• Assess residents' complex care and risk factors during the initial admission assessment period, and establish directives for ongoing care in conjunction with the resident/nominated representative
• Undertake and document evidence based assessment information promptly and clearly according to care needs and when care needs change
• Assess residents ongoing for complex care needs, anticipate residents' changing health needs and initiate further assessment or appropriate referral
• Plan with residents and/or their representatives and staff, goals and care in relation to complex health needs
• Provide skilled nursing care, and guidance and direction to other staff according to nursing care plans
• Perform functional responsibilities including medication administration, wound care, pain assessment and management, behaviour management and restraint management
• Communicate effectively with residents and their relatives/representatives, all levels of staff and other health care personnel
• Evaluate resident responses, side effects and effectiveness to all care interventions and recommend changes to care with the GP when appropriate
• Supervise and direct the on-the-job safety and health of subordinate staff
• Lead the clinical team allocated for the shift and represent the Executive after hours when holding the duties of RN in charge

About you

• Registered Nurse with current AHPRA registration
• Demonstrated clinical competency and currency as an RN
