Job summary

WA Primary Health Alliance (WAPHA) is recruiting an Activity Lead to oversee and coordinate health projects across Western Australia. The role focuses on service design, delivery, and system integration within primary care, working with governance bodies and providers.

You will lead planning, monitoring, and continuous improvement of activity models, across a two-year term with flexible/hybrid working from the Subiaco head office.

Qualifications

• Teritary qualification in health or human services or equivalent.
• Experience delivering end-to-end activities with planning, management and monitoring.
• Strong interpersonal and stakeholder engagement skills.
• Knowledge of WA health service delivery, primary care and general practice.
• Proficient in Microsoft Office; able to use common project tools.

Responsibilities

• Lead planning, commissioning, implementation and monitoring of activities with stakeholders.
• Monitor implementation, identify risks and opportunities, adjust for outcomes.
• Strengthen service integration and system partnerships for care pathways.
• Identify gaps to improve access, equity, integration and resource use.
• Develop and improve activity service models, processes and tools.
• Apply structured project methodology to create plans and monitor delivery.
