Job summary

Stream Services in Clermont, QLD is seeking compassionate Lifestyle Support Workers to support people with disability to live independently and participate in the community. The role offers flexible, part-time hours that can suit your schedule.

You’ll work with NDIS participants performing daily living activities, personal care, meal prep and household tasks while following tailored support plans and maintaining compliant documentation.

Qualifications

• Qualifications in disability support, aged care, community services – or equivalent experience as a carer.
• Experience providing person-centred support to NDIS or aged care clients (preferred but not essential).
• Current First Aid and CPR Certificates (or willingness to obtain).
• NDIS Worker Screening Check and Blue Card (or willingness to obtain).
• Full working rights in Australia.

Responsibilities

• Support NDIS Participants with daily living activities, including personal care, meal preparation and household tasks
• Provide social and recreational support to build confidence, independence and community participation.
• Develop and follow individualised support plans aligned with Participant goals
• Maintain accurate NDIS compliant documentation and reporting.
• Work collaboratively with the wider team to deliver person-centred, high-quality care.
