Job summary

Metro South Palliative Care Service is seeking a Palliative Care Social Worker to deliver compassionate psychosocial assessment and support to adults in palliative care across community and inpatient settings, including Canossa and Wynnum Manly Palliative Care Units.

You will provide comprehensive psychosocial assessments, educational support, and collaborate with multidisciplinary teams to coordinate care and ensure safe transitions.

Qualifications

• Compassionate and adaptable Social Worker with sound clinical judgement.
• Strong verbal and written communication skills and ability to collaborate within multidisciplinary teams.
• Experience with caseload management, prioritisation, problem-solving and independent working.

Responsibilities

• Provide comprehensive psychosocial assessments, therapeutic interventions and patient and family education.
• Support patients and families with complex end-of-life, emotional, social and bereavement needs.
• Collaborate with multidisciplinary teams and external providers to coordinate care and transitions between settings.
• Manage an allocated caseload, maintain clinical documentation and deliver timely service reporting.
