Health Workforce is seeking a Paediatric Coordinator based in Waterford West, QLD, to coordinate care for children (under 8, up to 16) and their families within an Aboriginal and Torres Strait Islander primary healthcare setting.

You will combine care coordination with direct clinical duties, triage referrals to paediatric services, deliver assessments, coordinate with internal and external stakeholders, and support immunisation programs and regular health checks in a culturally safe environment.

Qualificações

• Current AHPRA registration as a nurse.
• Current Queensland C Class Driver Licence.
• Current Queensland Disability Worker Screening Check (Yellow Card).
• Postgraduate qualification in Child Health, Public Health or a related field.
• Current First Aid and CPR certificates.
• Excellent communication and organisational skills.
• Experience working with Aboriginal and Torres Strait Islander people and/or communities is desirable but not mandatory.
• Demonstrated commitment to creating culturally safe environments for children and young people.

Responsabilidades

• Manage and triage referrals to paediatrician services.
• Deliver clinical services, including assessment of the needs of children and their families.
• Coordinate care throughout each child and family's engagement with the paediatrician service.
• Liaise with internal and external stakeholders, coordinate case conferences and maintain links with relevant services.
• Refer children and families to appropriate internal and external services to support their wellbeing.
• Deliver health promotion activities and support engagement with clinic staff and services, including immunisation programs and regular health checks.
