Job Overview

On Site Occupational Health Nurse – Injury Care & Wellness Programs

Key Responsibilities
  • Deliver patient-centred care within WA Home Therapies setting in accordance with organisational core values, standards of practice and APHRA guidelines.
  • Participate in clinical management, education and quality activities of staff and patients within training units.
  • Work autonomously within the centre with guidance from level 2 CNs and the WA Home Therapies Manager.
  • Design, plan, deliver and evaluate patient care within home dialysis therapies clinical setting, including home visits across WA, training patients/carers to manage peritoneal or haemodialysis.
  • Protect patient rights, maintain confidentiality by adhering to Privacy Principles.
  • Coordinate care for a group of dialysis patients, collaborating with tertiary and community health teams.
  • Educate and support clients to facilitate optimal health and well-being.
  • Support junior staff, including enrolled nurses, and work as part of a team to deliver excellent care.
  • Act as a preceptor and role model for staff education.
  • Promote values, mission and goals of the centre and company, working towards their achievement.
  • Promote a safe, secure and healthy working environment as detailed in the Occupational Health and Safety Site safety Plan.
  • Participate in quality improvement process by reporting incidents, hazards and assisting with audit opportunities.
  • Attend professional development activities to maintain and update professional knowledge and skills for clinical competence.
  • Complete clinical and mandatory competencies annually.
Management Functions (when requested)
  • Maintain accurate patient records and electronic patient records.
Qualifications & Key Skills
  • Registered as a general nurse with APHRA.
  • Demonstrated clinical competence and expertise (or working towards) in nephrology nursing.
  • Sound critical thinking skills and continuous reflection on practice.
  • Effective communication and interpersonal skills within a multidisciplinary team.
  • Organisational, decision-making and problem solving skills; self‑directed.
  • Ability to perform staff and patient education.
  • Knowledge of nursing, nephrology and other professional issues.
  • Completed post‑graduate Nephrology nursing theory and practice.
  • Computer literacy.
  • Commitment to quality improvement initiatives.
