The Transition Care Program (TCP) sits within the Operations Sub-Acute and Integrated Care cluster at Peninsula Care Group and provides a comprehensive range of multidisciplinary services across multiple sites on the Mornington Peninsula. The role provides leadership across clinical service delivery, supervision and education of Occupational Therapy staff and students, caseload and resource management, quality improvement and service development activities, while actively participating in both the TCP and Occupational Therapy Leadership Teams across Peninsula Health.
