Job summary

Henderson Healthcare in Melbourne, VIC is seeking an experienced Occupational Therapist to join our community aged care team. The role focuses on delivering client-centred services in clients' homes to help them stay safe, independent and living well.

You will manage a community caseload, conduct assessments, prescribe assistive technology, and work closely with families and the broader health team. AHPRA registration and Australian work rights are required; flexible hours and ongoing

Qualifications

• Experience with older adults in community, sub-acute, rehabilitation or transitional care settings is highly regarded.
• Current AHPRA registration as an Occupational Therapist is required.
• Confident working independently with a community caseload.
• Experience with seating and assistive technology/home modifications is desirable or willing to develop.

Responsibilities

• Conduct occupational therapy assessments in clients’ homes.
• Recommend assistive technology and home modifications to support safe and independent living.
• Complete functional assessments, clinical reports and clinical recommendations.
• Develop personalised intervention plans based on each client’s goals and needs.
• Work collaboratively with clients, families, support networks and the wider healthcare team.
• Manage your community caseload with a high level of autonomy.

About the Role:

Are you an Occupational Therapist looking for a rewarding community-based role where you can make a genuine difference to the lives of older Australians? We’re looking for an Occupational Therapist to join a supportive and collaborative team delivering community-based services across Melbourne.

You’ll work directly with clients in their own homes, providing practical, person-centred occupational therapy to help them remain safe, independent and living well in their own homes.

Your Caseload

• Home Care Package (HCP) clients
• Commonwealth Home Support Programme (CHSP) clients
• Functional assessments
• Falls prevention
• Assistive technology prescriptions
• Home modifications
• Activities of daily living and independence support

What You’ll Be Doing

About You

• Current AHPRA registration as an Occupational Therapist.
• Experience working with older adults in a community, sub-acute, rehabilitation or transitional care setting is highly regarded.
• Confident working independently and managing a community caseload.
• Experience with Seating & Home (SaH) and Assistive Technology & Home Modifications (AT-HM) is desirable, or a willingness to develop these skills.
• Current Australian driver's licence and access to a reliable vehicle.
