A leading health service provider in Queensland is seeking an Aboriginal and/or Torres Strait Islander Nurse Navigator to deliver advanced nursing care for children and adolescents. This role involves coordinating complex care journeys, partnering with families and health teams, and improving health outcomes. Candidates must possess current nursing registration and a passion for First Nations health. This is an identified vacancy with a genuine occupational requirement for the position to be filled by a candidate indigenous to the Aboriginal or Torres Strait Islander Community. Candidates not meeting this criteria will not be considered.
As an Aboriginal and / or Torres Strait Islander Nurse Navigator (Paediatric & Adolescent), you will deliver advanced, culturally safe nursing care and coordinate complex patient journeys across the continuum of care. Guided by Nurse Navigator principles, you will work in partnership with children, young people, families, and multidisciplinary teams to improve health outcomes, streamline care pathways, and strengthen system efficiency.
You are an experienced Registered Nurse who identifies as Aboriginal and / or Torres Strait Islander and are passionate about improving the health and wellbeing of First Nations children and young people.
