Job summary

Plena Healthcare is seeking a Dietitian to provide mobile, client-focused dietetics services to adults in Residential Aged Care facilities and the local community in Armidale, NSW. You will travel to facilities, deliver care, complete assessments, and contribute to education and program improvements.

Reporting to the Dietetics Team Leader, you will work with the Plena team to ensure high-quality care, rigorous documentation, and ongoing professional development within a values-led, supportive

Qualifications

• Current DA registration is required.
• Willingness to travel to residential aged care facilities.
• Ability to complete clinical documentation accurately.

Responsibilities

• Provide person-centred dietetics care to clients.
• Improve nutrition and hydration systems in aged care homes.
• Deliver educational sessions to staff and clients.
• Complete menu and mealtime assessments.
• Collaborate with external stakeholders and Plena colleagues.
• Maintain up-to-date clinical documentation.
