This ongoing part‑time role (37.5 hrs / fortnight) offers the chance to contribute to meaningful outcomes and work within a collaborative, multidisciplinary team.

Provide practical, day‑to‑day support to consumers, assisting them to achieve goals outlined in their care plans.

Support consumer independence, including housing, financial, social, and health‑related needs.

Collaborate with clinicians, families, and external agencies to ensure coordinated, recovery‑oriented care.

Contribute to team effectiveness and quality improvement by maintaining accurate records, supporting team communication, and participating in professional development.
