Job summary

Department of Health Tasmania in Hobart is seeking a Clinical Nurse Specialist – Discharge Planner to support safe, timely, and coordinated discharges within an acute mental health setting. This role operates within a multidisciplinary team to ensure person-centred, recovery-oriented discharge planning and to liaise with NDIS providers and accommodation services for smooth transitions.

Qualifications

• Demonstrated ability to deliver high-quality mental health care in an acute inpatient mental health setting with a particular focus on discharge planning assessments, risk assessments, brief psychological interventions and developing management plans.

Responsibilities

• Extensive demonstrated experience working in an inpatient mental health setting in an autonomous role and as part of a multidisciplinary team.
