Job summary

Amana Living in Perth is seeking a registered nurse to join the McCusker Nurse team, providing expert advice, support and referrals to carers of people living with dementia in metropolitan Perth from 8:30am to 4:30pm, 76 hours per fortnight, based at Subiaco corporate office.

You will build connections with dementia-specific agencies, advocate for clients, manage timely referrals, and educate aged care providers and community groups about dementia services.

Qualifications

• Registered nurse with extensive aged care experience.
• Experience supporting carers of people with dementia.
• Current APHRA Nursing Registration without restrictions.
• WA C Class Driver’s Licence and ability to drive locally.
• Ability to obtain National Police Clearance within 6 months.

The Role

The McCusker Nurse offers expert advice, support, and referrals to carers of people living with dementia in metropolitan Perth, regardless of their aged care provider. They act as a key resource, providing guidance and fostering connections with dementia specific agencies to provide support.

This position is being offered on a Full timebasis working 76hours per fortnight from 8:30am to 4:30pm based at our Corporate Office in Subiaco

Key responsibilities

• Provide dementia-specific person centred, information, education, and/or counselling, including telephone, written and face-to-face communication.
• Provide expert advice, and resources to support the carers of people living with dementia.
• Establish and maintain positive and professional relationships with people living with dementia, their carers, and the broader community.
• Provide advocacy, and support for people living with dementia and their caregivers.
• Ensure a timely and seamless referral process for clients to external and internal service providers
• Implement a regular survey which will collect, and report on outcome measure data from the service provided
• Maintain client record management and ensure reporting is in accordance with Amana Living’s policies and procedures.
• Where possible, provide education about dementia and McCusker Nurse services to Aged Care Service Providers, Aged Care Assessment teams, and other community groups.
• Present at professional conferences and workshops as requested, to promote and raise awareness of Amana Living’s role in dementia services.

About you

The ideal candidate will be an experienced registered nurse with extensive experience in the aged care sector, particularly supporting carers of people with dementia. They will possess strong knowledge of dementia and related services, along with excellent clinical reasoning and decision-making skills. Reporting to the McCusker Manager the candidate will be part of a team where they will have the skills to work autonomously, conduct telephone interviews, assess client needs effectively, and adapt to varying communication abilities. Proficiency in computer literacy, including Microsoft Office and other database programs, is essential.

• Evidence of Current Flu vaccinations per Amana Living policies
• Ability to obtain a National Police Clearance (within 6-months validity)
• Successful completion of pre-employment form and reference checksDegree in Nursing
• Current APHRA Nursing Registration without restrictions
• Western Australian C Class Driver’s License
