Job summary

Community Services #1 is seeking a casual Out of Hospital – Home Care Worker to deliver person-centred home care across ACT and surrounding NSW. The role focuses on assisting with domestic tasks, personal care and activities that support independence and wellbeing.

You will be expected to provide care across end-of-life stages, ensure safety, and document client progress while maintaining confidentiality. A Certificate III in Individual Support (Ageing) and current First Aid/CPR are required,

Qualifications

• Certificate III in Ageing or equivalent, plus 3+ months aged care experience.
• Current First Aid and CPR.
• Current Driver’s Licence with reliable vehicle and insurance.
• Able to obtain Police Check and Working with Children Check.

Responsibilities

• Deliver home care services aligned to Out of Hospital Care goals.
• Provide domestic assistance and personal care to support independence.
• Assist participants during end-of-life stages of care.
• Apply duty of care and safe work practices in client homes and document escalation triggers.
