Job summary

The ideal candidate is a Registered Nurse with AHPRA, capable of driving care planning, quality improvements and project initiatives within a large healthcare setting. This home offers supportive leadership and growth opportunities.

Qualifications

• Current registration as a Registered Nurse with AHPRA, no restrictions or conditions.
• Ability to build rapport and maintain relationships with key stakeholders.
• Strong understanding of aged care sector trends and best practices.
• Clear thinker with problem-solving skills; translate ideas into actions.
• Project coordination experience and ability to drive initiatives.
• Know-how in computerised systems used in the industry and adaptability to new systems.

Responsibilities

• Provide leadership and direction of the quality of care and experience across the home.
• Oversee assessments, care planning and case conferencing throughout the home.
• Ensure Care Plans are resident-owned and reviewed with families and carers.
• Oversee preadmission and admission processes for smooth transitions of care.
• Foster a culture enabling residents to make informed care decisions.
• Monitor high-risk residents and those with complex needs and changes in care.
