Job summary

Community Services #1 in Canberra is seeking casual Home Care Worker(s) to deliver high-quality, person-centred care that supports independence and wellbeing. You will provide domestic assistance, personal care, and support services to help clients maintain independence, including transport, appointments, and participation in community, leisure, and wellness activities.

Accurate daily shift notes, monitoring of client wellbeing and timely escalation of concerns to the Team Leader are essential,

Qualifications

• Certificate III in Individual Support (Ageing) or equivalent is required.
• 3+ months aged care experience.
• Experience delivering home care/aged care support (domestic assistance and/or personal care).

Responsibilities

• Provide domestic assistance, personal care, and support services that assist recovery and independence.
• Support clients with transport, appointments, communication, and participation in community, leisure, and wellness activities.
• Maintain accurate daily shift notes and monitor client wellbeing.
• Escalate concerns promptly to the Team Leader.
• Uphold dignity, privacy and confidentiality and collaborate with families and stakeholders.

About the role:

Provide domestic assistance, personal care, support services that assist recovery and independence.

Support clients with transport, appointments, communication, and participation in community, leisure, and wellness activities.

Maintain accurate daily shift notes, monitor client wellbeing, and promptly escalation any concerns to the Team Leader.

Ensure dignity, privacy, and respect in all interactions, while adhering to confidentiality, the Aged Care Code of Conduct, and collaborating with families and stakeholders.

About you:

Certificate III in Individual Support (Ageing) or equivalent, relevant experience delivering home care/aged care support (domestic assistance and/or personal care) and 3+ months aged carer experience.

Experience supporting clients with diverse cultural, emotional and health needs in community or out-of-hospital settings.

Ability to follow care plans and monitor client wellbeing.
