Job summary

Empower Healthcare is seeking a Home Care Physiotherapist for North Brisbane to deliver in-home physiotherapy across Support at Home and NDIS participants. You will assess, treat, and educate clients in their own homes while coordinating with a multidisciplinary team.

You will work in a supportive environment with opportunities to enhance clinical capability and leadership skills, along with flexible arrangements and strong CPD support.

Qualifications

• Must hold a Bachelor of Physiotherapy or equivalent.
• Registration with AHPRA.
• Police Check Clearance.
• Excellent verbal and written communication skills.

Responsibilities

• Assess participants and deliver tailored physiotherapy treatment plans for home-based care.
• Collaborate with other allied health professionals to develop multidisciplinary care plans.
• Educate participants and families to maintain healthy living in the community.
• Liaise with service providers to ensure clear communication and coordination.
• Work autonomously to prioritise tasks, maintain records and provide patient reports.

The Position

Our innovative service model focuses on delivering the best clinical outcomes for participants. As a Physiotherapist with Empower, you will have the opportunity to work with a variety of Musculoskeletal Neurological and Cardiorespiratory patients. Working in a supportive team environment, you will be responsible for the delivery of Empower Healthcare's comprehensive services across Support at Home and NDIS participants in North Brisbane.

• Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for participants in their own home through Support at home packages/NDIS.
• Work holistically with other allied health professionals to provide a multidisciplinary care plan
• Educate participants and their families to optimise their ability to live healthy lives within the community
• Liaise with service providers to ensure effective communication
• Work autonomously to prioritize and complete tasks, maintain records and provide patient reports

You will be provided with support from our home care team who will schedule these appointments and plan an efficient route.
