Job summary

Southern Cross Care is seeking a Home Care Coordinator (Enrolled Nurse) in Darwin to be the central point of contact for clients. You will ensure services run smoothly according to plans and budgets, conduct assessments, and deliver nursing care within EN scope.

You will coordinate daily service requests, assist with equipment and vendor bookings, and support clients from diverse backgrounds. A valid nursing diploma, AHPRA registration, and own car are essential.

Qualifications

• Diploma of Nursing with current unrestricted AHPRA registration.
• First Aid (HLTAID011) and CPR (HLTAID009) certificates required.
• Current Australian driver’s license and your own registered, insured car.

Responsibilities

• Conduct client assessments, reviews and onboarding with a health-promoting focus.
• Provide nursing services within the Scope of Practice for Enrolled Nurses.
• Coordinate daily check-ins, service requests, equipment purchases and vendor bookings.
• Support clients from Culturally and Linguistically Diverse and Aboriginal and Torres Strait Islander backgrounds.
• Travel to clients within the region using your own registered, insured car.

About the role

About you

We are looking for people who will be the difference in the lives of our residents and clients.

The successful candidate will have the following;

• A Diploma of Nursing and current unrestricted AHPRA registration.

• Current First Aid (HLTAID011) and CPR (HLTAID009) Certificates.

• A current Australian driver’s licence and your own registered, insured car.

• Demonstrated experience as an Enrolled Nurse (EN) in aged care or home care.

• Strong communication skills and basic knowledge of Aged Care Standards and My Aged Care systems
