About the role

This newly created position—Head of Service Delivery, Direct Care—is a strategic and operational leadership role responsible for the quality, effectiveness, and growth of our in‑home aged care and transitional care services.

Reporting to the General Manager, Client Services, you will lead the transformation and expansion of our direct care model, ensuring services are person‑centred, financially sustainable, and aligned with our new model of care. You will shape workforce structures, embed best practice, and build service capability to meet the evolving needs of older people and those transitioning between care settings.

This is a high‑impact role at a pivotal time, offering the opportunity to design how services are delivered nationally and influence the future direction of our organisation.

About you

We’re looking for a highly capable leader who brings:

  • Significant experience leading service delivery in aged care, community services, health or related sectors, delivering results in complex environments
  • Deep understanding of person‑centred care and its operational application
  • Proven ability to lead multidisciplinary teams through organisational change and service redesign
  • Strong knowledge of aged care funding frameworks, compliance, and quality systems
  • Exceptional communication, partnership building and problem‑solving skills
  • Strategic thinking, analytical capability, and a passion for innovation
  • Tertiary qualifications in Health, Aged Care, Community Services, Management or Business Administration
  • Experience managing subcontracted service models, transitional care, hospital discharge pathways or integrated care environments
  • Postgraduate qualifications in Leadership, Business Administration or Change Management
  • Relevant qualifications in person‑centred care, Dementia care or quality systems.

To be considered for this role you must have full Australian working rights, current Police & Working with Children Checks – we can arrange police checks for successful candidates. Our selection process also includes psychometric assessment.
