Job summary

Inglewood & Districts Health Service in Mildura, VIC invites applications for a Grade 1 Mental Health Practitioner / Social Worker on a fixed-term contract. You will undertake psychosocial assessments, develop recovery-oriented care plans, provide brief interventions, and collaborate with families

Qualifications

• Approved tertiary qualification in Social Work and eligibility for AASW.
• Understanding of mental health, psychosocial disability and AOD.
• Ability to undertake psychosocial assessment and identify needs and risks.
• Commitment to recovery-oriented and trauma-informed practice.

Responsibilities

• Undertake psychosocial, mental health and AOD assessments with consumers.
• Develop recovery-oriented, person-centred care plans with consumers.
• Provide evidence-informed brief interventions and psychosocial support.
• Support progress toward goals in mental health, AOD and social wellbeing.
• Monitor progress, identify risks, escalate concerns when needed.
• Collaborate with multidisciplinary team and community services.
• Develop knowledge through supervision and reflective practice.
