Job summary

My Community Health is seeking a Community Physiotherapist to join our established In-Home Nursing Team serving the Central Coast. You will provide comprehensive, high-quality physiotherapy services to the local community, coordinating with clients, families, care workers and health professionals.

This role focuses on independence, wellbeing and meaningful client-centred care, with opportunities for career progression as the team grows.

Qualifications

• Tertiary qualification in Physiotherapy and current AHPRA registration.
• Ability to communicate effectively with clients, families and multidisciplinary teams.
• Commitment to evidence-based practice and continual professional development.

Responsibilities

• Conduct thorough initial assessments and develop customised treatment plans.
• Deliver physiotherapy interventions including manual therapy, exercise prescription and rehab programs.
• Collaborate with clients, families and other health professionals to ensure coordinated care.
• Maintain accurate client records, complying with policies and regulations.
• Participate in team meetings and professional development opportunities.
• Champion a client-centred approach to meet individual goals.

What you'll be doing?

• Conduct thorough initial assessments to develop customised treatment plans for each client.

• Deliver a range of physiotherapy interventions, including manual therapy, exercise prescription, and rehabilitation programs

• Collaborate with clients, their families, and other healthcare professionals to ensure seamless, coordinated care

• Maintain accurate and up-to-date client records, ensuring compliance with all relevant policies and regulations

• Participate in regular team meetings and professional development opportunities to stay informed of industry best practices

• Champion a client-centred approach, advocating for the unique needs and goals of each individual

What are we looking for?

• Tertiary qualification in Physiotherapist and current registration with the Australian Health Practitioner Regulation Agency (AHPRA)

• Demonstrated ability to effectively communicate and build strong relationships with clients, families, and multidisciplinary teams

• Excellent clinical reasoning and problem-solving skills, with a commitment to evidence-based practice

• Passion for promoting health, wellbeing, and independence in the local community

• Flexible and adaptable approach, with the ability to work collaboratively in a team environment

• Must have Australian working rights
