Job summary

The Queensland Government, through the Forensic Disability Service, seeks a committed professional to support clients with life skills and community reintegration. You will develop, implement and evaluate plans, monitor risks, and help clients advance toward personal goals within their IDP.

As part of the operational services team, you will contribute to assessments, transition plans from secure facilities and liaise with clinical staff while maintaining confidentiality and safety standards.

Qualifications

• Certificate IV in Communities or approved equivalent, or ability to undertake within prescribed timeframe.
• Experience developing and evaluating support plans to improve life skills.
• Ability to monitor at-risk clients and report to clinical teams.

Responsibilities

• Develop, implement and evaluate client support plans to improve life skills and transition back to the community.
• Monitor at-risk clients and communicate risk information to the clinical team.
• Support clients to achieve goals in their Individual Development Plan and promote community reintegration.
• Implement treatment and habilitation programs in consultation with clinical staff.
• As part of the team, contribute to risk assessments and transition plans from secure facilities to the community.
• Prepare documentation and maintain confidentiality, including reports and data collection.
• Ensure safety of clients, staff and visitors and participate in managing challenging behaviour.

Your Mandatory Requirements

• Certificate IV in Communities or approved equivalent or ability to undertake a Certificate IV in Communities within a prescribed timeframe
• Possession of a ‘C’ class driver licence
• Possession of a current ‘Provide First Aid’ certificate (or ability to obtain a certificate prior to commencement of employment)
• Develop, implement and evaluate support plans to assist clients to improve and maintain life skills thereby increasing positive behaviours, improving quality of life and working towards a transition back to the community. This will include supporting clients to develop skills required for independent everyday living such as leisure activities, meal preparation and cleaning.
• Monitor and observe clients who are at risk to self and others, able to identify and provide relevant information to the clinical team surrounding risk concerns.
• Support each client to achieve the goals within their Individual Development Plan (IDP), to promote community reintegration.
• Implement treatment programs developed by clinical services staff to address specific offending behaviours.
• Implement habilitation programs in consultation with Operational Team Leaders and the Clinical team.
• Support clients to develop personal routines and lifestyles to meet their individual needs, abilities and preferences, by:
• communicating and interacting with the client in ways that are respectful, culturally sensitive, encourage personal choice and protect rights and dignity
• identifying and acting on opportunities for clients to develop skills and competencies
• facilitating clients to maintain relationships with family, friends and support networks as identified in their individual development plan
• effectively responding to and supporting clients to develop positive alternatives to engaging in offending behaviour and/or behaviours of harm and
• actively seek to identify opportunities with local community organisations, sport and recreation clubs and educational and vocational providers to support clients to maximise access for clients to a range of opportunities.
• As a member of the operational services team, actively contribute to the development, implementation and review of:
• client assessments including risk assessments and venue risk assessments
• individualised plans including complex health support plans, medication support plans and individual development plans
• limited community treatment opportunities and plans
• transition plans from the secure facility back to the community.
• Prepare reports, maintain documentation, and undertake administrative tasks and data collection as required, while maintaining confidentiality requirements.
• Ensure the safety of clients, staff and visitors and the security of the facility including observations and searches, duress response duties and participate in the management of challenging and aggressive behaviour.
• It is vital that Forensic Officers have the ability to be emotionally resilient and demonstrate perseverance in achieving objectives and coping effectively with setbacks, challenges and incidents.
• Support clients to access the community as well as supporting them on overnight Limited Community Treatment to link with supports and resources in their own community with a view of transfer and transition.
• Adhere to legislation, policies, procedures and practices and work within the legislative framework that governs the Forensic Disability Service.
• Adhere to the decisions made by the Operational Team Leaders, and follow direction provided by members of the Senior Leadership Team.
• Participate in identifying, developing and implementing continuous improvement initiatives to improve quality of support to clients and service delivery.
• Support clients with taking medicines according to the directions prescribed by a suitably qualified medical practitioner.
• You may also exercise specific powers or functions as conferred by a declaration, appointment or delegation under the Forensic Disability Act 2011.
