Job summary

Omnia Community Health invites applications for a compassionate Family Services Practitioner to join the Loddon Family Services team. This part-time, fixed-term role focuses on delivering child-centred, strengths-based and culturally responsive practice to children, young people and families.

You will provide early intervention, case management and wraparound support, working with vulnerable families to build safety and stability while reducing statutory involvement.

Qualifications

• Compassionate and skilled Family Services Practitioner with a strengths-based approach.
• Experience in case management and working with complex family needs.
• Ability to collaborate with multiple partners and services.

Responsibilities

• Provide early intervention, case management and intensive wraparound support.
• Assess needs and risks, develop Family Action Plans, and deliver practical/therapeutic support.
• Collaborate with Child Protection, The Orange Door, education, health and community partners.

Education

Bachelor qualification in Social Work, Psychology, Counselling or another human services related discipline
