Job summary

Lumia Care Pty is seeking a compassionate Enrolled Nurse (Care Partner) to deliver high-quality, person-centred in-home care across client homes in South Australia. You will provide hands-on clinical support, monitor wellbeing, and maintain accurate documentation to ensure safe, consistent care.

Ideal candidates hold an Enrolled Nurse qualification, current police clearance, WWCC, NDIS worker screening, a valid driver’s licence with reliable vehicle, and up-to-date immunisations.

Qualifications

• Requires qualification as Enrolled Nurse and current registration.
• Police clearance and WWCC required or willing to obtain.
• NDIS Worker Screening Clearance (or willingness to obtain)
• Valid driver's licence and reliable vehicle
• Immunisations up to date and full right to work in Australia

Responsibilities

• Deliver personal care and in-home support per care plans.
• Monitor client wellbeing and escalate concerns per pathways.
• Contribute observations to care plan development and review.
• Support implementation of care plans with safe, person-centred practices.
• Maintain accurate, timely clinical documentation for compliance.
