Job summary

Each is seeking a compassionate Support Worker for a casual, office-based role in Port Macquarie with travel to surrounding areas. A minimum Cert III in Aged Care or Individual Support is required, and experience with older adults is valued.

You'll help clients live independently at home by delivering personalised support. Responsibilities include implementing individual service plans, providing companionship, assisting with personal care, and helping with transport to appointments.

Qualifications

• Minimum Cert III in Aged Care, or Individual Support (must have completed aged care components).
• Experience working with older adults.
• A valid driver’s licence.

Responsibilities

• Deliver personalised support to help older adults maintain independence at home.
• Implement individual care plans, healthcare plans, and positive behaviour support strategies.
• Provide companionship, conversation and personal care in the client’s home.
• Assist with transportation to important appointments and travel between clients.
