A respected Aboriginal Medical Service in Broome is looking for Elder Care Connectors to assist Aboriginal Elders in navigating aged care services. Qualifications

  • Experience in aged care, community services, disability support, My Aged Care navigation, or working within an Aboriginal Community Controlled Health Organisation will be highly regarded.
  • You do not need formal qualifications – lived experience and community knowledge are welcomed.

Responsibilities

  • Engaging with Elders and families to raise awareness of aged care services.
  • Supporting Elders with My Aged Care registration and assessments.
  • Assisting with goal setting, care coordination, and follow-up.
  • Documenting activities and case notes accurately.

Skills

Experience engaging with Aboriginal Elders, families, or community members
Understanding of Aboriginal culture, values, and community dynamics
Strong communication and relationship-building skills
Confidence working independently while contributing to a team
Basic computer skills and ability to complete documentation
Organisational skills and the ability to manage priorities
