Job description

In these Local Area Coordination roles, working in the region of Pialba, you will be responsible for assisting people with disability, their families, and their carers to build and pursue their goal to live their best life, exercise choice and control, and engage with the National Disability Insurance Scheme – Partners in Community.

Responsibilities
  • Assisting people with ‘permanent and significant’ disability to apply for NDIS supports.
  • Collaborating with participants to develop goals
  • Connecting people with disabilities to support within the local community
  • Supporting participants to navigate and optimise their engagement with the NDIS
  • Improving community knowledge of the NDIS and creating new opportunities to connect participants to services and support
  • Meeting identified contractual KPIs

Strong personal values based on empathy and compassion, you understand the diverse needs of individuals within your local community and can coach, counsel, and connect people to identify and achieve the positive outcomes they desire.

Requirements
  • Demonstrated relevant experience and / or lived experience
  • Experience in planning with people to achieve collaborative outcomes
  • Knowledge of the local community and the available options for social & economic participation for people with disability
  • Demonstrated computer skills including capability with Microsoft Office 365
  • Currently hold or able to hold a Working with Children and or a Vulnerable People Check
