Job summary

Darling Downs and West Moreton PHN – Springfield QLD is seeking a Director to drive the transformation of primary care across our region and help shape a more connected, accessible and responsive health system. The role leads strategic planning, program design and governance to ensure evidence-based decisions, population health alignment and service improvements.

Qualifications

• Extensive experience in leading complex and long-term change in health or community settings.
• Proven ability to lead strategic planning and performance accountability.
• Strong leadership and people management skills with stakeholder collaboration.

Responsibilities

• Provide strategic and operational leadership to enable primary care transformation and workforce planning.
• Lead development and implementation of Healthy Primary Care initiatives and health system integration.
• Oversee primary care team operations, coaching, and performance management.
• Collaborate with government, NGOs, and health partners to address service gaps and access issues.
• Report on program delivery, budget, HR, and risk across the Primary Care Team.
