Job summary

Murrumbidgee Local Health District in Wagga Wagga welcomes an experienced Dietitian to join the Transitional Aged Care Program. In this role you will assess nutrition needs, develop individualized care plans and deliver targeted interventions to support recovery and independence for older adults returning home from hospital.

You’ll work within a multidisciplinary team, manage a caseload across up to 25 TACP packages (subject to hours) and conduct home visits to coordinate care with clients,

Qualifications

• A tertiary qualification in nutrition and dietetics with eligibility for full membership with the Dietitians Australia (DA) including Accredited Practising Dietitian (APD) status.
• Ability to work as a member of a multidisciplinary team.
• Commitment to quality improvement and evidence-based practice.
• Previous experience in a clinical setting including general medical / surgical units and / or community-based nutrition and multi-disciplinary team services.

Responsibilities

• Provide comprehensive nutrition assessments and develop individualized nutrition care plans.
• Deliver tailored nutrition interventions and review outcomes to support recovery and independence.
• Manage a diverse caseload and conduct home visits to address nutritional needs and goals.
• Collaborate with clients, carers and other service providers as part of a multidisciplinary team.
