Job summary

Amana Living is seeking a compassionate Coordinator to lead the Pause Program for people living with dementia and their carers, based at our Subiaco head office. You will coordinate person-centred respite and wellbeing to support living well at home.

Working with clients, families and stakeholders, you will develop care plans, deliver education, maintain records and ensure funding compliance, while helping grow the dementia respite model across the program.

Qualifications

• Qualified health professional with a relevant field (allied health or nursing).
• Experience supporting people living with dementia and carers.
• Current flu vaccination as per Amana Living policy.
• Ability to obtain National Police Clearance (within 6 months).
• Pre-employment checks including reference checks completed.

Responsibilities

• Lead the Pause Program delivery and develop person-centred respite plans.
• Engage with participants (carers and people living with dementia) to ensure timely respite support.
• Conduct assessments to tailor informal and formal respite options.
• Maintain records and report outputs to funding requirements.
• Co-design respite planning tools and collaborate with stakeholders.
• Build community partnerships to enhance dementia respite services.
