Job summary

Embedded Health Solutions (EHS) is seeking an experienced pharmacist to deliver comprehensive RMMRs and HMRs across aged care facilities in Australia. You will conduct QUM services, attend MAC meetings, advise on medication use and storage, and maintain strong relationships with facility staff and GPs.

Qualified candidates will hold an undergraduate pharmacy degree with unconditional AHPRA registration, professional indemnity, and compliance with WorkCover, ABN and police clearances.

Qualifications

• Credentialing certificate to perform medication management review services.
• Recognised undergraduate pharmacy degree and unconditional AHPRA registration as a pharmacist in Australia.
• Professional indemnity and WorkCover insurance policies.
• Australian Business Number (ABN) and Australian citizen or permanent residency status.
• Police clearance and statutory declaration where required.
• Annual flu and COVID-19 vaccination (where not exempt).
• Working with Children Check/NDIS clearance if required.
• Proficiency in computing applications and strong clinical skills.
• Excellent organisational and time management abilities.
• Interest in building relationships with healthcare professionals in aged care.
• Effective communication with residents, relatives and healthcare staff.
• Personal laptop.

About you
