Qualifications

• Understanding of case management components: assessment, planning, service coordination and brokerage.
• Ability to provide constructive feedback and respond to criticism.
• Strong attention to communication and non-verbal cues to promote inclusion.
• Proactive problem solving and independent action when needed.

Responsibilities

• Apply order administration activities to service users.
• Refer individuals to relevant services and monitor compliance against order requirements.
• Implement appropriate responses to non-compliance and elevate matters as required to senior staff in a timely manner.
• Produce case notes, reports and other standard documents in line with procedural guidelines.
• Work collaboratively with colleagues, contribute to team planning, work process improvements and day-to-day administration of local work area.
