Sage GroupHealthCare Staffing is seeking a Registered Nurse in Home Health for Marion, OH. This travel/contract position offers a 13-week engagement with 40 hours per week, day shifts, and on-call requirements.

Responsibilities include developing and implementing care plans, coordinating in-home nursing services, and ensuring accurate charting and compliance with OASIS and state regulations. Must have an Ohio RN license, BLS, and home health experience.

Qualifications

• RN with 2 years of Home Health experience required
• Recent/Home Health Nursing experience 1 year in last 3 years required
• OASIS experience 1 year required
• OASIS admissions and Case Management required
• Home Care Home Base (HCHB) charting experience preferred
• Timely communication and documentation required
• Must have valid driver’s license and auto insurance

Responsibilities

• Provide and coordinate care for assigned patients in the home.
• Develop and implement Start of Care and admission nursing care plans.
• Support patient recovery to remain at home with family involvement.
• Educate patients and families on disease management.
• Deliver nursing care per state Nurse Practice Act and policy.
• Ensure legible physicians' orders and proper transcription.
• Assess patient status and document changes promptly.
• Notify physicians of significant changes in status.
• Coordinate with office and complete charting promptly.
