Job summary

St John of God Health Care is seeking a compassionate coordinator to deliver meaningful palliative care programs within a multidisciplinary team. You will engage patients, carers and families through therapeutic and creative activities, ensuring holistic, person-centred care across the continuum.

The role involves collaboration with clinicians and community providers, mentoring volunteers, and supporting events such as Palliative Care Week.

Qualifications

• Understanding the needs of palliative care patients and carers.
• Excellent communication and interpersonal skills.
• Compassionate team player with multidisciplinary collaboration experience.

Responsibilities

• Plan, coordinate and evaluate engaging therapeutic, recreational and creative programs for patients, carers and families.
• Collaborate with the interdisciplinary team to support holistic, person-centred palliative care and care planning.
• Provide individual support, follow-up care and complementary therapies within scope of practice to enhance patient wellbeing.
• Build and maintain strong relationships with community organisations, health providers and professional networks to support seamless care.
• Promote Support and Therapy services and contribute to community engagement, education and service awareness initiatives.
• Support and mentor volunteers, students and community placements in the delivery of palliative care programs and activities.
• Coordinate and contribute to special events, celebrations and awareness activities, including Palliative Care Week initiatives.
• Monitor patient and family wellbeing, communicating observations and concerns to the multidisciplinary team to support quality care outcomes.
